- Lab Requisition for 12-hour Fasting Bloodwork
- Virtual Report of Findings with Dr. Plank
- Functional Wellness Report FWR (see pictures above+)
- Supplement & Lifestyle Recommendation Report
Must be completed fasting at a QuestDiagnostics Blood Draw Center Only
*You must be a PA resident and have the bloodwork completed in PA. No retirement or Medicare health insurance plans are accepted. We will need your birthdate and address to complete this lab requisition. Please allow 2 business days (orders M-Th) for us to get your information. Your lab results and FWR will be emailed to you 15 minutes before your virtual report of findings with Dr. Plank, no exceptions. We will contact you to schedule your lab review and lab report of findings after we receive your results and Dr. Plank has had time to review your labs. You may incur charges depending on your insurance policy limits and coverage. We take no responsibility for charges not covered by your insurance when you chose this service.
The labs needed will be determined by Dr. Plank after your New Client Appointment (Schedule New Client Appointment Now)
There are no refunds issued once the lab requisition has been shared with you.
